The reliability of motor evoked potentials to predict dorsiflexion injuries during lumbosacral deformity surgery: Importance of multiple myotomal monitoring

Lieberman JA, et al. - Experts ascertained the sensitivity and specificity of motor evoked potential (MEP) to predict isolated nerve root injury causing dorsiflexion weakness in selected patients having complex lumbar spine surgery. In order to predict a new postoperative dorsiflexion injury, the use of MEP amplitude change is highly sensitive and specific. Findings suggested the superiority of monitoring two myotomes (both tibialis anterior (TA) and extensor hallucis longus (EHL)) over relying on MEP changes from a single myotome. They noted less accuracy of EMG activity but that it complimented the use of MEP.
