The prognosis of streptococcal prosthetic bone and joint infections depends on surgical management, a multicenter retrospective study

Mahieu R, et al. - Via retrospectively reviewing a cohort of streptococcal prosthetic joint infections (PJIs) in seven reference centers for management of complex bone and joint infections between January 1, 2010 and December 31, 2012, researchers sought for the optimal treatment of streptococcal PJIs. They included 70 patients with monomicrobial infections, with 47 infections in total hip arthroplasty and 23 infections in total knee arthroplasty. Observations revealed a poor prognosis for streptococcal PJIs managed with debridement, antibiotic and implant retention (DAIR). A trend toward benefit was noted only in association with DAIR with polyethylene exchange (PE) compared to DAIR without PE. S. agalactiae species seems to be an independent factor of treatment failure. A better outcome was not linked to rifampicin or levofloxacin in combination.
