Tachyphylaxis during treatment of exudative age-related macular degeneration with aflibercept

Hara C, Wakabayashi T, Fukushima Y, et al. - In this retrospective, interventional, consecutive case series involving 313 patients (313 eyes), researchers examined the features of patients who developed tachyphylaxis [defined as lack of any improvement (more than 100 μm) in or deterioration of CRT within 1 month after more than two repeated monthly IVA injections when the exudative change remained] on repeated intravitreal aflibercept (IVA) injections for neovascular age-related macular degeneration (AMD) and the frequency of tachyphylaxis. The study sample consisted of patients with treatment-naïve AMD who achieved resolution soon after starting IVA and were followed up for ≥ 12 months. According to findings, tachyphylaxis occurs in a minority of long-term AMD patients following repeated IVA injections and is more likely to occur in eyes with lesions under the retinal pigment epithelium and without intraretinal edema. Treatment of AMD should be performed whilst considering the consecutive treatment, taking this fact into account.
