Risk factors and outcomes after surgical reconstruction of charcot neuroarthropathy in fracture vs dislocation patterns

Cates NK, Furmanek J, Dubois KS, et al. - This research sought to compare the preoperative risk factors and postoperative outcomes between Charcot neuroarthropathy patients with dislocation vs purely fracture pattern breakdown. Researchers further compared the same factors between Charcot neuroarthropathy patients with dislocation vs fracture-dislocation pattern breakdown. The study enrolled a total of 55 patients with the forefoot, midfoot, or hindfoot Charcot Neuroarthopathy at a mean follow-up of 2.99 years. Bivariate analysis was conducted to compare preoperative risk factors and postoperative outcomes, and segmented multivariable regression analysis was conducted. It was shown that involvement of the talonavicular joint, in the fracture pattern Charcot associated with medial column collapse occurring at the onset of Charcot breakdown. In Charcot Neuroarthropathy, the dislocation pattern demonstrated a higher propensity for residual collapse as indicated by the higher rates of broken hardware, osteomyelitis, and the need for revisional exostectomy.
