Repeated oral glucose tolerance tests in women at risk for gestational diabetes mellitus

de Wit L, et al. - As clinical signs or symptoms suggestive for gestational diabetes mellitus (GDM) may develop in the third trimester among pregnant women with a negative oral glucose tolerance test (OGTT) between 24-28 weeks, researchers examined the additional yield of repeating an OGTT to detect missed GDM in this group. In addition, they appraised patient characteristics and indications that were associated with a positive second OGTT. They performed a retrospective cohort study of 3,147 women at risk for GDM. A second OGTT was performed on 183 of 3,147 women in the third trimester following their regular OGTT at 24-28 weeks and identified GDM in 43 women (23.5%). This indicates that OGTT repetition after initial negative screening leads to additional GDM diagnoses. Consideration of repeating an OGTT could be given in case of clinical signs, especially in women with additional risk factors such as a history of GDM or higher index OGTT glucose values.
