Relationships between orthostatic hypotension, frailty, falling and mortality in elderly care home residents

Shaw BH, et al. - Among 116 older adults living in long-term care facilities, researchers assessed the links between orthostatic hypotension (OH), frailty, falling, and mortality. They generated a frailty index (FI-MDS), using the Minimum Data Set (MDS) document, from a list of 58 deficits, ranging from 0 (no deficits) to 1.0 (58 deficits). A 15-minute supine-seated orthostatic stress test was performed to determine OH. In these subjects, FI-MDS enabled the evaluation of frailty, which was associated with proneness to OH, falling risk, and 3-year mortality. For these individuals, a simple and convenient risk assessment tool could be an FI-MDS. A particularly high risk of falling was observed among older adults who were both frail and had impaired orthostatic blood pressure control.
