Prophylactic gastrostomy in locally advanced head and neck cancer: Results of a national survey among radiation oncologists

Dragan T, Duprez F, Van Gossum A, et al. - Since the use of a prophylactic gastrostomy (PG) tube to prevent malnutrition in head and neck cancer (HNC) patients receiving definitive concurrent chemoradiotherapy (CCRT) has received a lot of attention in recent literature, but the appropriate guidelines in clinical practice have not yet been established, researchers sought to explore the current PG tube utilization among Belgian radiation oncology centers. A survey was sent to all 24 Belgian Radiation oncology departments, asking about the number of patients treated each year, whether the PG indication is discussed at the multidisciplinary board, placement technique, time of starting nutrition and removal, its impact on swallowing function, and importance of clinical factors. This survey confirms that the decision to place and use a PG tube in the context of locally advanced HNC patients undergoing CCRT is a complicated process with widely varying clinical practice. The standardization of recommendations and clinical guidelines is an absolute necessity. Additional solid research is required to support better, evidence-based clinical practice.
