Progression of behavioural disturbances across frontotemporal dementia: A longitudinal observational study

Cosseddu M, Benussi A, Gazzina S, et al. - In a large cohort of patients with behavioural variant frontotemporal dementia [FTD] (bvFTD), nonfluent/agrammatic variant primary progressive aphasia (nfvPPA) and semantic variant PPA (svPPA), researchers explored behavioural symptoms at baseline and during the course of the disease. For this longitudinal observational study, 403 FTD patients were analyzed, 167 of which had at least one-year follow-up evaluation (for a total of 764 assessments). Through the FTLD-Clinical Dementia Rating scale (FTLD-CDR), disease severity was assessed. According to results, patients with bvFTD showed more behavioural disturbances than with nfvPPA at baseline. Negative symptoms (apathy and empathy loss) showed a trend towards a rise in both bvFTD and PPA throughout the course of the disease. In the advanced stages, positive symptoms (disinhibition, perseverations and hyperorality) seem to improve, emphasizing the importance of taking into account the severity of the disease as a measure of outcome in clinical trials.
