Prognosis and its predictors after incident stroke in patients With type 1 diabetes

Hagg–Holmberg S et al. – This study determined the predictors of survival after incident stroke in patients with type 1 diabetes and concluded that such patients have a poor cardiovascular prognosis and a high risk of all–cause mortality. Hemorrhagic stroke and progression of diabetic kidney disease are associated with worse outcome.
Methods
- This observational study included 144 patients with type 1 diabetes who suffered a stroke between 1997 and 2010 from Finnish Diabetic Nephropathy (FinnDiane) Study.
- Hard–cardiovascular events and death as a result of cardiovascular– or diabetes–related cause was collectively referred to as vascular composite endpoint.
Results
- A total of 104 (72%) patients suffered a vascular composite endpoint: 33 had a recurrent stroke, 33 a hard–cardiovascular event, and 76 died of cardiovascular– or diabetes–related causes.
- Survival rates were 76% and 58% at 1–year and 5–year follow–up, respectively.
- Hemorrhagic stroke and chronic kidney disease stages 2, 3, 4, and 5 were predictors of a vascular composite endpoint.
- Worsening kidney function increased all–cause mortality.
