Preeclampsia and risk of end stage kidney disease: A Swedish nationwide cohort study

Khashan AS, Evans M, Kublickas M, et al. - Using data from the Swedish Medical Birth Register, women who had singleton live births in Sweden between 1982 and 2012, involving those who had preeclampsia (n = 1,366,441 healthy women who had 2,665,320 singleton live births), were recognized by the researchers in order to investigate the correlation between preeclampsia and the risk of end-stage kidney disease (ESKD) in healthy women, while taking into account preexisting comorbidity and potential confounders. Women who had preeclampsia in at least one pregnancy were fivefold more prone to have ESKD in comparison with parous women who had never had preeclampsia. This relationship was independent of several sociodemographic factors like maternal age and education and prepregnancy comorbidities like renal disease and cardiovascular disease. in conclusion, preeclampsia and no major comorbidities prior to their first pregnancy were at a 5-fold increased risk of ESKD vs parous women with no preeclampsia. In women with preeclampsia, the absolute risk of ESKD prevailed to be small. Moreover, for subsequent ESKD, preeclampsia should be acknowledged as a significant risk factor. In women with adverse pregnancy outcomes, whether screening and/or preventive strategies would decrease the risk of ESKD required more investigation.
