Predicting long-term renal and patient survival by clinicopathological features in elderly patients undergoing a renal biopsy in a UK cohort

Navaratnarajah A, et al. - In this retrospective cohort study, 463 subjects of age >70 years old who underwent a renal biopsy between 2006 and 2015 were analyzed to assess the renal and patient consequences according to the indication for biopsy, clinical parameters, and the histological diagnosis. They noted pauci-immune crescentic glomerulonephritis (GN; 12%), acute interstitial nephritis (10.8%) and membranous GN (7.1%) as the most frequent primary diagnoses. They recorded factors linked to the risk of progression to ESRD: creatinine, heavy proteinuria and non-chronic kidney disease (CKD) biopsy indication. An association of a histological diagnosis of primary GN or tubulointerstitial nephritis with a favorable renal outcome was noticed. They reported the highest risk of requiring dialysis among subjects with vasculitis and paraprotein-related renal disease (PPRD). They found PPRD as an independent risk factor for death.
