Pediatric Surgical Wait Priority Score (pSWAPS): Modifying a health system's adult-based elective surgery prioritization system for children's surgery during the COVID-19 pandemic

Slater BJ, Cappello MT, Butterly MM, et al. - Perioperative concerns, transmission risk, and the necessity to divert resources with the rise of COVID-19 cases led societies to recommend cancellation of all elective surgical procedures. There appeared a necessity for a system to triage and prioritize scheduling operations once the number of cases stabilized. Construction of a universal scoring system to triage surgical elective cases was done for the Advocate Aurora Health system (Surgical Wait Priority Score, SWAPS) and its modification was done for use in pediatrics (pSWAPS). Significant contributors to the pWAPS were 18 factors, creating a cumulative score ranging from 0 to 120. pSWAPS system is identified as a simple, flexible scoring system that considers resource constraints. Use of pSWAPS has been made for 2 months. Observations suggests this system as an effective tool for safe and methodical reintroduction of elective procedures during the COVID-19 pandemic and its ability to be used again for another surge.
