Outcomes associated with catheter ablation of VT in patients with cardiac sarcoidosis

Siontis KC, Santangeli P, Muser D, et al. - Catheter ablation combined with medical therapy can play a crucial role in the management of cardiac sarcoidosis (CS)-associated ventricular tachycardia (VT), including in cases with VT storm.
This is a multicenter study of 158 patients with CS-associated VT undergoing catheter ablation, to determine results linked with VT ablation in CS.
In 85 cases (54%), complete procedural success (no inducible VT postablation) was obtained.
Significant reduction in antiarrhythmic drug need as well as in defibrillator shocks was evident, and VT storm was eliminated in 82% of patients with that presentation.
One- and 2-year rates of survival free of VT recurrence, heart transplant, or death were estimated to be 60% and 52%, respectively.
Adverse long-term prognosis postablation was observed in relation to presence of LV dysfunction and myocardial inflammation.
A favorable long-term outcome was noted in relation to a history of hypertension.
