Outcome of status epilepticus and the predictive value of the EMSE and STESS scores: A prospective study

Sairanen JJ, et al. - Between March 23 and December 31, 2015, consecutive adults with status epilepticus (SE) in the Kuopio University Hospital emergency department were selected in order to evaluate the short-term consequence of status epilepticus (SE) and examine the Epidemiology-based Mortality score in Status Epilepticus (EMSE) and the Status Epilepticus Severity Score (STESS) performance in outcome anticipation. According to this prospective study, 151 SE episodes in 137 patients were recorded, of whom 47 had a first-time epileptic event (seizure or SE). The area under receiver operating characteristic curves of the EMSE-EAC (EMSE using the combination of etiology, age, and comorbidity) and STESS for mortality prediction were 0.790 and 0.736, respectively. Negative predictive mortality values were 97.5% and 96.7% respectively using EMSE-EAC-34 and STESS-4. The authors discovered that more than 40% of patients with SE suffer from adverse outcomes. With a high negative predictive value, EMSE-EAC and STESS are useful in predicting short-term mortality. For the cohort, the optimized cutoff points for the EMSE-EAC and STESS were ≥ 34 and ≥ 4.
