National estimates of CKD prevalence and potential impact of estimating glomerular filtration rate without race

Duggal V, Montez-Rath ME, Chertow GM, et al. - Researchers investigated alterations in CKD prevalence as well as the potential impact on therapeutic drug prescriptions and prediction of renal failure if race adjustment were eliminated from the CKD-EPI GFR estimating equation. From adults aged ≥18 years in the National Health and Nutrition Examination Survey and the Veterans Affairs Health Care System, cross-sectional and longitudinal data were obtained. Findings revealed that the estimated prevalence of CKD, among Black adults in the United States, would be doubled if race adjustment were removed from CKD-EPI eGFR. A sizeable number of drug-dosing decisions are likely to be impacted by such a change. It may also result in improved accuracy of renal failure risk prediction in higher-risk Black adults.
