Misdiagnosis worsens prognosis in subarachnoid hemorrhage with good Hunt and Hess Score

Ois A, Vivas E, Figueras-Aguirre G, et al. - Researchers conducted this prospective cohort study to characterize variables correlated with initial misdiagnosis, defined as failure to identify correctly, of subarachnoid hemorrhage (SAH), as well as examined the association of misdiagnosis with poor outcome and complications in good Hunt and Hess (HH) cases. Participants in the study were 401 patients with SAH. The authors discovered that misdiagnosis was confirmed in 104/401 patients who also had longer periods of hospital admission. The poor outcome has been recorded in 167/401 patients. Findings revealed that misdiagnosis is an unresolved SAH problem, and in patients with HH 1–2 it is a missed opportunity for a good outcome. Independent predictors of poor outcome were age, misdiagnosis, and greater clinical and radiological initial severity. The poor outcome in misdiagnosed patients was partly explained by a higher risk of delayed cerebral ischemia and complications associated with the procedure. The diagnostic approach needs to be improved in patients who record headache only (HH 1–2) after SAH.
