Meta-analysis comparing outcomes and need for renal replacement therapy of transcatheter aortic valve implantation vs surgical aortic valve replacement

Siddiqui WJ, et al. - Researchers analyzed data from randomized control trials (RCT) comparing Transcatheter Aortic Valve Implantation (TAVI) to surgical aortic valve replacement (SAVR) in terms of short and long-term impact on renal outcomes in patients with severe aortic stenosis. Primary outcomes were acute kidney injury (AKI) within 30 days and 1 year of TAVI, and requirement for renal replacement therapy (RRT). Statistical analysis was carried out using RevMan version 5.3. A total of 5,536 patients (2,796 in TAVI and 2,740 in SAVR arm) from 6 RCTs were included in the analysis. Compared with SAVR, reduction in AKIs at 30 days was noted in relation to TAVI, despite the exposure to contrast and higher incidence of new permanent pacemaker placement noted with TAVI.
