Long-term surgical outcomes of ab externo trabeculotomy in the management of primary congenital glaucoma

Esfandiari H, et al. - Via retrospectively reviewing the records of pediatric patients operated on between September 2006 and June 2018, researchers examined the long-term outcomes of ab externo trabeculotomy with a Harms trabeculotome at a single, tertiary care pediatric hospital. Using Cox proportional hazards ratio, risk factors for failure were identified. Of 40 patients, a total of 63 eyes were included. Data reported that the cumulative likelihood of achievement rate was 83% at 3 months, 76% at 6, 73% at 12, 72% at 18, and 65% at final visit. Findings revealed that the presentation was linked to a less favorable outcome within 3 months of life. Eighteen months after surgery, intraocular pressure and success rate remained stable. Overall, the authors concluded that ab externo trabeculotomy was linked to good long-term outcomes. Better long-term success rates were associated with more extensive trabeculotomy (ie, more than one procedure.
