Left ventricular diastolic dysfunction and diseases severity contribute to impaired exercise capacity in systemic lupus erythematosus

Ciurzyński M, Chrzanowska A, Bienias P, et al. - Among patients suffering from systemic lupus erythematosus (SLE), this inquiry was conducted to assess potential link between exercise capacity evaluated by six minute walk test (6MWT) and echocardiographic parameters of left and right ventricular function. A prospective analysis of 66 SLE patients and 27 age matched healthy individuals was conducted. In both groups, E/A (LV diastolic dysfunction parameter) was similar. SLE patients exhibited higher E/E’ (LV diastolic dysfunction parameter) vs controls. In SLE cases, a significantly shorter mean 6MWT distance was revealed. In univariable logistic regression models, a link with 6MWT distance lower than < 450 m was shown by SLICC/ACR-DI, E/E’, tricuspid regurgitant peak gradient, and right ventricular systolic pressure. In this study, severity of SLE and LV diastolic dysfunction appeared to be the main cause of impaired exercise tolerance.
