Incidence and prognosis of primary gastrinomas in the hepatobiliary tract

Norton JA, et al. - The incidence and prognosis of unusual gastrinomas originating in the hepatobiliary tract were determined. An uncommon (3%) occurrence of primary gastrinomas of the hepatobiliary tract was shown; however, the hepatobiliary system was the second most frequent extraduodenopancreatic primary location after the lymph nodes. Tumors were shown to occur outside the gastrinoma triangle. In addition, the operative approach was changed when aggressive liver or bile duct resection was indicated, with high rates of long-term cure and survival and acceptable rates of complications. Routine excision of the lymph nodes in the porta hepatis was indicated because nearly 50% of patients had lymph node metastases.
