Impact of dialysis on the prognosis of patients undergoing transcatheter aortic valve implantation
American Journal of Cardiology Oct 24, 2018
Schymik G, et al. - Researchers investigated the prognostic impact of dialysis in patients with end-stage renal disease (ESRD) undergoing transcatheter aortic valve implantation(TAVI) for severe aortic stenosis (sAS). They analyzed procedural and longer-term outcomes for 2,000 patients (dialysis group, n=56 [2.8%]) who were consecutively enrolled between April 2008 and March 2015 when diagnosed with sAS and were eligible to undergo TAVI. The analyzed outcomes were adjusted for differences in baseline characteristics. According to findings, dialysis was identified as an independent predictor of mortality in patients undergoing TAVI. Kidney disease/dialysis itself appeared to be largely determining the long-term mortality in dialysis patients while no impact, to a large extent, was observed on VARC2 defined complications. (Pre-) procedural optimization was still necessary on account of an increased short-term mortality.
