How does screw migration or fracture after anterior cervical plate fixation affect the radiographic and clinical outcomes?

Park S, Lee DH, Ha JK, et al. - Via performing a retrospective cohort study researchers investigated how screw migration and fracture associated with anterior cervical plating influence long-term radiographic and clinical outcomes. For this work, they reviewed medical records and radiographic data of 248 consecutive patients who underwent anterior cervical discectomy and fusion or anterior cervical corpectomy and fusion with a dynamic plating system and were followed up for ≥ 2 years. Of these, 25 patients experienced screw migration or breakage and were included in screw failure group; the nonfailure group comprised 223 patients who reported no screw loosening or fracture until the last follow-up. Observations revealed a significant association of a number of levels fused with an increased risk of screw failure. Screw failure did not aggravate postoperative arm pain, neck pain, or neck disability despite its relation with an increased incidence of pseudarthrosis. Immediate removal is rarely required, as the migration of failed implants rarely occurs to an extent that endangers tracheoesophageal structures.
