HIV-associated comorbidities as mediators of the association between people living with HIV and hospital-acquired infections

Rojas D, et al. - The relationship between HIV and hospital-acquired infections (HAIs) was determined as were the possible mediating effects of comorbidities on this association were investigated. Using the Louisiana Hospital Inpatient Discharge Database for the years 2011-2015, researchers included all patients with at least 1 HAI diagnosis as cases in the case-control study. They randomly selected at a 1:1 ratio controls from the same hospitals. Among 1,852,769 eligible hospital discharges from the study period, 7,422 patients had at least 1 HAI. As per marginal logistic regressions, there was a robust correlation between HIV and central line–associated bloodstream infections (CLABSIs); there was an inverse correlation between HIV and any HAI. Mediation analyses revealed that having at least 1 comorbidity mediates the relationship between HIV and CLABSIs. These findings indicate an increased risk of developing a CLABSI among people living with HIV, similar to the published literature.
