Fortification of breast milk with preterm formula powder vs human milk fortifier in preterm neonates: A randomized noninferiority trial

Chinnappan A, Sharma A, Agarwal R, et al. - In this open-label, noninferiority, randomized trial, experts aspired to explore whether fortification of expressed breast milk (EBM) by preterm formula powder is noninferior to fortification by human milk fortifiers (HMF), in terms of short-term weight gain, in very-low-birth-weight (VLBW) neonates. From December 2017 to June 2019, this trial was performed at a level 3 neonatal unit in India. The trial included preterm (born before 34 weeks of gestation) VLBW neonates who received at least 100 mL/kg/d of feeds and consumed at least 75% of milk as EBM. Of the 123 neonates registered, 60 were assigned to the PTF group and 63 to the HMF group, respectively. In preterm neonates, fortification with preterm formula powder is not inferior to fortification with human milk fortifiers. The preterm formula may be a better choice for fortification, particularly in resource-constrained settings, due to the potential reduction in feed intolerance and lower costs.
