Evolution of anosognosia in Alzheimer disease and its relationship to amyloid

Hanseeuw BJ, Scott MR, Sikkes S, et al. - In view of the fact that a challenging manifestation of Alzheimer disease is unawareness, or anosognosia, of memory deficits, that has adverse influences on a patient's safety and decision-making, researchers analyzed alteration in awareness of memory abilities as well as its link with beta-amyloid (Aß) burden in a large cohort (N = 1,070) of people across the disease spectrum. They performed a longitudinal evaluation of memory awareness (average number of visits = 4.3) as well as the operationalization using the discrepancy between mean participant and partner report on the Everyday Cognition scale. They used [18F]Florbetapir positron emission tomography imaging to determine Aß deposition at baseline. Findings revealed a link of Aß burden with a progressive reduction in self-awareness of memory deficits, reaching anosognosia nearly three years prior to dementia diagnosis.
