Jeemon P, Harikrishnan S, Ganapathi S, et al. - A family-based cardiovascular risk reduction intervention performed in individuals with a family history of premature coronary heart disease resulted in reduction of total cardiovascular risk. Such an approach can encourage the adoption of lifestyle modifications to decrease cardiovascular risk in high-risk families.

• This open-label, cluster randomised controlled trial (PROLIFIC) included families of individuals with coronary heart disease who had been diagnosed before age 55 years.

• Of 750 families, 368 (with 825 participants) were randomized to the intervention group and 382 (with 846 participants) to the usual care group.

• The intervention group received a comprehensive package of interventions (screening, structured lifestyle interventions, linkage to a primary health-care facility, and active follow-up for adherence).

• Primary outcome was defined as achievement or maintenance of any three of the following: blood pressure < 140/90 mm Hg, fasting plasma glucose < 110 mg/dL, low-density lipoprotein cholesterol < 100 mg/dL, and abstinence from tobacco.

• At 2-year, 64% participants in the intervention group and 46% in the usual care group achieved the primary outcome.

• Two times higher odds of achieving the primary outcome at the 2-year timepoint was noted in the intervention group, vs usual care group, post-adjustment for clustering and baseline risk factors.
