Effect of preoperative tricuspid and/or mitral regurgitation on development of late right-sided heart failure after insertion of the HeartWare left ventricular assist device

Gonzalez-Fernandez O, Bouzas-Cruz N, Ferrera C, et al. - In this retrospective analysis, researchers examined all adult patients who received HeartWare-ventricular assist device implantation as a bridge to transplantation in a single-centre, to define the factors predictive of late right-sided heart failure (RHF) in left ventricular assist device patients. RHF needing rehospitalization following 30 days of implantation defined late RHF. The development of late RHF was reported in 16 (10.3%) patients from 156 implantations. Median time to late RHF onset was 182.5 days. Patients presenting with late RHF had a significantly higher rate of moderate or severe tricuspid regurgitation prior to implantation. A strong link with late RHF was shown by many echocardiographic parameters at discharge postimplant, such as significant mitral regurgitation. According to the findings, the predictors of the occurrence of late RHF were preoperative significant tricuspid regurgitation and mitral regurgitation following implantation.
