Drowning classification: A reappraisal of clinical presentation and prognosis for severe cases

Markarian T, Loundou A, Heyer V, et al. - Given Szpilman introduced a classification of drowning in 1997 that has become the reference, and it appeared suitable to reevaluate the prognosis as well as clinical presentation of drowned patients more than 20 years following this first publication, so, researchers undertook this retrospective analysis in Intensive Care Units located in France, French Polynesia, and French Antilles, to afford a reappraisal of patients who required advanced health care as well as to provide an accurate depiction of their respective neurological, respiratory, and hemodynamic profiles. There were 312 drowned patients who got admitted with severe clinical presentation (Grade 2–6). Speedy extraction from the water (< 10 min for all) and specialized care (Emergency Medical Services), commencing from the prehospital period, was found to be beneficial for all patients. As per the findings, drowning-associated cardiac arrest continues to be the prognosis cornerstone. Better prognosis than previously expected was reported for other victims. This reinforces the value of specialized intervention to interrupt the drowning process.
