Does internal fixation for unstable osteochondritis dissecans of the skeletally mature knee work? A systematic review

Leland DP, et al. - Researchers examined data of patients who were skeletally mature and underwent internal fixation of an unstable unstable osteochondritis dissecans lesion of the knee for the rate of radiographic union, patient-reported outcomes, complications, and reoperations. Searching literature according to Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines resulted in the identification of 13 studies with 148 patients (150 knees; 14 to 45 years) for inclusion. Use of numerous fixation procedures was reported, including biodegradable devices (rods, pins, nails, and screws), metal screws, Kirschner wire, and autologous bone sticks. Outcomes revealed the achievement of acceptable rates of radiographic union and improved patient-reported outcomes with performing internal fixation of the native fragment vs other techniques.
