Do we need 30 min cortisol measurement in the short synacthen test: A retrospective study

Kumar R, et al. - In this single-center retrospective study of 393 short synacthen test (SST) measuring 0, 30 and 60 min cortisol levels following administration of 250 µg of synacthen, experts examined whether both 30 and 60 min sample were needed after the administration of synacthen to examine suspected adrenal insufficiency (AI). Results imply that if the 60 min sample is not sustained, about 5% of individuals undergoing SST could be inappropriately diagnosed as having AI (and constrained to long-term undesirable steroid treatment). It was recommended that the 30 min sample does not add any supplementary diagnostic utility and could be eliminated therefore making SST easy even further and saving on cost and resources. Also, single measurement following 60 min of administration of synthetic ACTH was proposed to be an adequate screening test for AI.
