Diagnosis and outcome of acute respiratory failure in immunocompromised after bronchoscopy

Bauer PR, et al. - Researchers focused on the risks and benefits of bronchoscopy in immunocompromised patients with acute respiratory failure admitted to ICU. In this pre-planned secondary analysis of Efraim (a prospective, multinational, observational study), 618 (39%) patients who had bronchoscopy were included. Overall 165 patients were diagnosed with the help of bronchoscopy alone (27% adjusted diagnostic yield). In 69 (11%) of patients, worsening of respiratory status was noted in relation to bronchoscopy. The overall rate of undiagnosed causes was identified to be 13%. Findings revealed an association of bronchoscopy with improved diagnosis and changes in management but also with increased hospital mortality.
