Comparative safety of antipsychotic medications in elderly stroke survivors: A nationwide claim data and stroke registry linkage cohort study

Su CC, Yang YHK, Lai ECC, et al. - Despite reports describing an increased risk of mortality, antipsychotics remain the first choice of treatment for post stroke psychosis in elderly patients. The mortality risk was compared among antipsychotics in elderly patients with stroke using the stroke registry for external adjustment. A retrospective cohort study was conducted in the National Health Insurance Database (NHID) to recognize patients aged above 65 years who were admitted for stroke from 2002 to 2014. Haloperidol, quetiapine and risperidone were prescribed to 22 235, 28 702 and 8 663 stroke patients, respectively. Findings suggest that relative to risperidone and haloperidol, quetiapine would pose less mortality risk at doses higher than 0.5 defined daily dose among post stroke elderly patients. In cases with indications for haloperidol or risperidone, they suggest starting with a lower dose to avoid excess risk.
