Clinical significance of genotyping for human papillomavirus (HPV) 16 18/45 combined with cytology in cervical exfoliated cells in HPV oncogenic mRNA-positive women

Wang J, et al. - In this study performed on 3,257 women aged 25-65 years, researchers evaluated the clinical significance of human papilloma virus (HPV) 16 18/45 typing test results plus cytology for cervical exfoliated cells from women who screened positive in an HPV E6/E7 mRNA assay (Aptima HPV, AHPV). The participants were AHPV-positive and had AHPV 16 18/45 Genotype assay (AHPV-GT) testing with cytology. Comparison of different triaging strategies was carried out. A total of 624 women (19.2%) tested AHPV-GT positive. A similar AUC but a higher specificity and positive predictive value (PPV) were seen with AHPV-GT vs cytology, when identifying CIN2+. Overall, in women receiving AHPV-positive primary screening tests, the AHPV-GT test could be a promising triage approach with high specificity. The same CIN2+ and CIN3+ cases were identified when a combination strategy and co-testing strategy were implemented but significantly fewer screening tests were needed for the former.
