Characterization of rapid cycling bipolar patients presenting with major depressive episode within the BRIDGE‐II‐MIX study

Furio MA, Popovic D, Vieta E, et al. - The “Bipolar Disorders: Improving Diagnosis, Guidance, and Education” (BRIDGE‐II‐Mix) study was conducted to determine the frequency of mixed states in patients with a major depressive episode (MDE) according to different definitions. In addition, a comparison was performed concerning their clinical validity, looking into specific features such as rapid cycling (RC). Among 2,811 assessed MDE patients, 726 (25.8%) received a diagnosis of bipolar disorder (BD), and DSM‐5 criteria for RC were met by 159 (21.9%) of these BD patients. Findings suggest that those in the BD‐RC group had a greater number of lifetime depressive episodes with shorter duration of depressive episodes, and more psychiatric comorbidities, as well as higher rates of atypical features and concomitant (hypo)manic symptoms (irritable mood; risky behavior; impulsivity; and psychomotor agitation). Patients with RC had a worse functioning, more obesity, and were significantly more likely to be treated with three or more drugs.
