Changing demographics and risk factors for cryptococcosis: A 12‐year review at a tertiary care centre

Mycoses Jun 16, 2021

Bhatt M, Porterfield JZ, Ribes JA, et al. - Cryptococcosis is classically linked with the immunocompromised patients, however, a rising appreciation for its impact on the immunocompetent hosts has been evident. Researchers herein conducted a retrospective study of 12 years of patients with cryptococcosis with the aim to examine the trends, diagnosis, treatment of different hosts and the effect of immunodeficiency and chronic liver disease on relapse and in-house mortality. Among 114 identified patients, 23 (20.2%) had HIV infection, 11 (9.6%) had transplant, 80 (70.2%) were non-HIV non-transplant (NHNT) patients. Overall, there was a mortality rate of 28.1% (32/114) and occurrence of relapse in 10.5% (12/114) of patients. Of all studied patients, chronic liver disease represented 30.7% (35/114) and was a risk factor for in-hospital mortality. They did not identify HIV infection and transplant as statistically significant for mortality. The HIV-infected patients showed the highest relapse at 30.4% (7/23). These data highlight that cryptococcosis is affected by the type and degree of immunocompromise.
