Cardiopulmonary exercise testing provides additional prognostic information in cystic fibrosis

Hebestreit H, et al. - By analyzing retrospectively gathered data from 10 cystic fibrosis (CF) centers in Australia, Europe, and North America, researchers assessed the prognostic significance of cardiopulmonary exercise testing (CPET)-derived measures beyond peak oxygen uptake (V.o2peak) after rigorous adjustment for other predictors. At 10-year follow-up, V.o2peak in % predicted, peak work rate, ventilatory equivalent for oxygen, and carbon dioxide were identified as significant predictors of death or lung transplantation, as revealed by Cox regression after adjustment for sex, FEV1% predicted, body mass index (z-score), age at CPET, Pseudomonas aeruginosa status, and CF-related diabetes as covariates in the model. In CF, CPET offered additional prognostic information to established predictors of death/lung transplantation. Regular monitoring of exercise capacity and exercise counseling could be especially beneficial for high-risk patients.
