Association of acute increases in plasma creatinine after renin-angiotensin blockade with subsequent outcomes

Fu EL, Trevisan M, Clase CM, et al. - In this retrospective analysis, researchers used an extensive population-based cohort to compare health outcomes among patients with various classes of increase in creatinine upon the start of renin-angiotensin system inhibition (RASi). They used the Stockholm CREAtinine Measurements database. Participants were adults (n = 31,951) starting RASi during 2007–2011 with available pre- and postinitiation creatinine monitoring. These individuals were observed for a median duration of 3.5 years. Worse health outcomes were reported in correlation with increases in creatinine (> 10%) following initiation of RASi among real-world monitored adults. The issue of cessation of RASi, when plasma creatinine increases, was not addressed but higher subsequent risk of cardiovascular and kidney outcomes was found in patients exhibiting increases in creatinine.
