Are Descemet membrane ruptures the root cause of corneal hydrops in keratoconic eyes?

Parker JS, et al. - Researchers estimated 16 eyes of 10 candidates to assess the mechanism for corneal hydrops in eyes with keratoconus (KC) in retrospective, interventional case series. Uncomplicated Descemet membrane endothelial keratoplasty (DMEK) was done in 16 eyes of 10 cases with comorbid KC and Fuchs endothelial corneal dystrophy (FECD) whereas 5 eyes of 5 subjects with KC alone underwent Bowman layer (BL) transplantation, complicated by inadvertent perforation of the posterior corneal stroma and Descemet membrane (DM). They estimated the presence or absence of hydrops by intra- and postoperative optical coherence tomography and by slit-lamp biomicroscopy. They observed no manifestation of hydrops at any time during or after surgery. But, the combined defect in DM and the posterior corneal stroma was consistently exciting typical corneal hydrops.
