All‐cause and cause‐specific mortality in psoriatic arthritis and ankylosing spondylitis: A systematic review and meta‐analysis

Chaudhary H, Bohra N, Syed K, et al. - Ankylosing spondylitis (AS) but not psoriatic Arthritis (PsA) carries a significantly elevated risk of overall mortality. For both these conditions, a higher cardiovascular-specific mortality was observed, highlighting the importance of early screening and management of cardiovascular risk factors.
A higher risk of cardio-metabolic comorbidities is seen in PsA and AS vs the general population.
A systematic review and meta‐analysis of 19 studies documenting all-cause or cause-specific mortality in patients with PsA and AS than the general population (11 studies of PsA, 7 of AS, 1 of both).
In PsA studies, mortality was not higher than the general population (relative risk RR: 1.12, 95% CI: 0.96-1.30, 10 studies).
For PsA patients, significantly higher were cardiovascular, respiratory, and infection specific mortality risks (RR: 1.21, 3.37 and 2.43, respectively) but not cancer-related mortality (RR: 1.01).
Relative to the general population, a higher risk of mortality due to all causes (RR 1.64, 95% CI: 1.49-1.80, 6 studies) and cardiovascular causes (RR 1.35, 95% CI: 1.01-1.81, 3 studies) was reported in AS.
