Age and frailty influence hip and knee arthroplasty reimbursement in a bundled payment care improvement initiative

Pepper AM, et al. - A sum of 1821 candidates were retrospectively analyzed to assess the impact of patient age and frailty on reimbursement after hip and knee total joint arthroplasty (TJA) from 2013 to 2016. They estimated the age as a categorical variable, with the median age of the sample population as the categorical cutoff. They noted a relation of age ≥72 years and increasing modified frailty index (mFI) score with statistically significant elevated cost. They presented a statistically significant increase in the cost of 0.68% per incremental age increment with reference to increasing age. Statistically significant progress in cost for mFI score ≥2 was observed while multivariate evaluation of increasing age and mFI. They reported the over-simplification of the cost compared with TJA in the Bundled Payment Care Improvement (BPCI) initiative.
