Which postoperative day after total joint arthroplasty are catastrophic events most likely to occur?

Johnson DJ, et al. - Through 341,601 patients who underwent primary total joint arthroplasty (TJA) between 2011 and 2016 were recognized in the National Surgical Quality Improvement Program by the researchers in order to spread an awareness of the timeframe for most common catastrophic complications that could help providers to maintain an appropriate degree of suspicion for significant events. Pulmonary embolism (PE), myocardial infarction or cardiac arrest, cerebrovascular accident, and death were 4 possible postoperative outcomes that were assessed. One thousand six hundred thirty-one with a postoperative PE, 1,033 patients with a cardiac event, 304 patients with a cerebrovascular accident, and 608 expired patients were observed. Out of all these complications, more than 60% of them occurred between postoperative days 0 and 5, with 65% of PE. In comparison to PE, a cardiac event was 13 times more likely to be the catastrophic event on postoperative day 0. Hence, for certain patients undergoing TJA, risk assessment for catastrophic events and timeliness of occurrence could guide providers to suggest enhanced observation.
