Ventilation time and prognosis after stroke thrombectomy: The shorter, the better!

Fandler-Höfler S, Heschl S, Kneihsl M, et al. - The researchers examined the clinical impact of the duration of artificial ventilation in 447 stroke patients who underwent treatment at their center with mechanical thrombectomy (MT) for anterior circulation large vessel occlusion under general anesthesia over an 8-year period. Ventilation time was assessed as a continuous variable and the patients were grouped into extubation within 6 hours ("early"), 6-24 hours ("delayed") and > 24 hours ("late"). The mean age of patients was 69.1±13.3 years; 50.1% were females; median ventilation time was 3 hours. Outcomes suggest the independent predictive value of prolonged ventilation time after stroke thrombectomy for the unfavorable outcome at 3 months and its association with increased pneumonia rates. Therefore, they emphasize performing extubation as early as safely possible.
