Value of routine timed barium esophagram follow-up in achalasia after myotomy

Kachala SS, et al. - In this study, researchers sought to assess the value of routine timed barium esophagram (TBE) in longitudinal follow-up of achalasia after Heller myotomy. They recognized follow-up TBE as valuable post-myotomy, particularly if there is substantial esophageal dilatation preoperatively. Follow-up TBE provided reassurance to patients with stable or decreasing TBE measures, permitting decreased follow-up intensity. Because a successful, single, non-surgical reintervention often results in long-term successful palliation, they recommended not considering reintervention a myotomy failure. More than 1 reintervention required intensification of TBE follow-up, facilitating treatment planning.
