Validation of the Rotterdam Elderly Pain Observation Scale in the hospital setting

Boerlage AA, et al. - In view of the utility of Rotterdam Elderly Pain Observation Scale (REPOS) in assessing pain in noncommunicative and cognitively impaired nursing home residents, researchers evaluated the reliability and validity of this scale for pain assessment in the hospital setting. They performed a prospective multicenter observational study including in total 72 patients; 118 observations of surgical and 68 observations of internal medicine patients were analyzed. They assessed pain behavior from the video recordings with the REPOS and the Pain Assessment Checklist for seniors with Severe Dementia—Dutch language (PACSLAC-D). Using numeric rating scale score of ≥4 as the reference value, a REPOS cutoff score of ≥3 had optimal sensitivity (78%) and specificity (90%) for the detection of pain. Outcomes suggest that for hospital patients who cannot self-report pain, REPOS is reliable and valid for postoperative and chronic pain assessment.
