Utility of routine use of breast ultrasound following contrast-enhanced spectral mammography

Klang E, et al. - Researchers investigated if breast ultrasound (US) is routinely indicated following contrast-enhanced spectral mammography (CESM). The retrospective collection of consecutive screening and diagnostic CESM examinations with concurrent breast US (May 2012 to February 2016) was followed by the assignment of a separate Breast Imaging-Reporting and Data System (BIRADS) score for CESM and for US by radiologists. They estimated the links between malignant pathology with either suspicious-appearing CESM or suspicious-appearing US. They also analyzed the sensitivities and specificities of CESM and US. They concluded that it is appropriate to perform subsequent US and biopsy when CESM is suspicious appearing. Findings suggested a correlation with US with a CESM BIRADS 3. If the CESM is benign appearing, the routine use of US may result in unnecessary benign biopsies and hence is questionable.
