Utility of contrast‐enhanced harmonic endoscopic ultrasonography for predicting the prognosis of pancreatic neuroendocrine neoplasms

Ishikawa R, Kamata K, Hara A, et al. - Researchers conducted this single‐center, retrospective study to examine the ability of contrast‐enhanced harmonic endoscopic ultrasonography (CH‐EUS) to predict the prognosis of pancreatic neuroendocrine neoplasms (PanNENs). The sample consisted of 47 consecutive patients who were diagnosed with PanNEN by surgical resection or EUS‐guided fine needle aspiration between December 2011 and February 2016 and had CH‐EUS. According to the degree of clinical malignancy, patients were divided into aggressive and non‐aggressive groups, resulting in 19 aggressive PanNEN cases and 28 non‐aggressive PanNEN cases. Results indicated that hypo‐enhancement on CH‐EUS was linked to smaller and fewer vessels and a greater degree of fibrosis, after assessment of 36 resected specimens. Overall, the authors concluded that CH‐EUS can be useful for anticipating the prognosis of PanNENs. For the diagnosis of aggressive PanNEN, CH‐EUS was better than contrast‐enhanced computed tomography. Hypo‐enhancement on CH‐EUS was an indicator of aggressive PanNEN; among G1/G2 PanNENs, cases with hypo‐enhancement on CH‐EUS had a worse prognosis vs those with hyper/iso‐enhancement.
