Using the risk analysis index to assess frailty in veteran cohort undergoing endovascular aortic aneurysm repair

Tse W, et al. - In view of the utility of the Risk Analysis Index (RAI), a validated frailty score system, in predicting short-term outcomes and long-term mortality in various surgical subspecialties, researchers here applied the frailty score to the veteran aneurysm population. Querying Veteran Affairs Surgical Quality Improvement Program data, they identified 5,878 patients (99.5% male; average age: 71 ± 8 years) who underwent endovascular repair of infrarenal abdominal aortic aneurysm or dissection. Observations revealed a dose-dependent relationship between frailty as determined by the RAI and postoperative outcomes. Higher rates of major cardiac (myocardial infarction, cardiac arrest), pulmonary (pneumonia, failure to wean vent, reintubation), renal (renal insufficiency, renal failure), overall complications, and death were observed in correlation with frailty. Based on findings, researchers here recommended the utility of this frailty index as a screening tool to guide discussions with patients undergoing endovascular aortic aneurysm repair.
