Using high-sensitivity cardiac troponin for the exclusion of inducible myocardial ischemia in symptomatic patients a cohort study

Walter J, du Fay de Lavallaz J, Koechlinet L, et al. - Researchers performed a prospective diagnostic cohort study to use a novel approach applying very low concentrations of high-sensitivity cardiac troponin I (hs-cTnI) for the exclusion of inducible myocardial ischemia in symptomatic patients with coronary artery disease (CAD). A total of 1,896 consecutive individuals with CAD referred with symptoms possibly related to inducible myocardial ischemia were included in this study. The inducible myocardial ischemia was noted in 865 individuals (46%). It was reported that the hs-cTnI cutoff of 2.5 ng/L gave an NPV of 70% and a sensitivity of 90% for the exclusion of inducible myocardial ischemia. Very low hs-cTn concentrations, including hs-cTnI concentrations less than 2.5 ng/L, do not generally permit users to carefully exclude inducible myocardial ischemia, in symptomatic individuals with CAD.
