Usefulness of calculation of cardiovascular risk factors to predict outcomes in patients with acute myocardial infarction

Kim CY, et al. - Given the differential contribution of cardiovascular risk factors to short-term prognosis of acute myocardial infarction (AMI), researchers determined if an enhanced prediction of major adverse cardiac and cerebrovascular events (MACCEs) in post-MI patients could be achieved by a simple calculation of conventional risk factors, PARADOCS (Pressure of ARtery elevAtion, Diabetes, Obesity, Cholesterol, Smoking) score. From Korean AMI Registry – National Institute of Health database, a total of 13,104 AMI patients were selected and analyzed. Patients with 1-year MACCEs vs those without MACCEs exhibited significantly greater PARADOCS score. After adjusting for confounding variables, PARADOCS score was identified as an independent predictor of 1-year MACCEs in Cox proportional hazards model. Worse clinical outcome was reported in patients with greater PARADOCS score, in Kaplan-Meier survival curve. Overall, findings suggested the possible utility of PARADOCS score in offering valuable prognostic information of MI patients to clinicians, however, further validation is required.
