Use of nephrotoxic medications in adults with chronic kidney disease in Swedish and US routine care

Bosi A, Xu Y, Gasparini A, et al. - Researchers sought to describe the use of nephrotoxic medications in patients with CKD Stage G3-5 in routine care.
Researchers assessed cohorts of adults with confirmed CKD G3-5 undergoing routine care in the two health systems (Stockholm CREAtinine Measurements [SCREAM], Stockholm, Sweden [N = 57,880]; and Geisinger, Pennsylvania, US [N = 16,255]).
Nephrotoxic medications were administered to one in five patients with CKD in the two distinct health systems.
NSAIDs, antivirals and immunosuppressants were the top nephrotoxic medications identified in both cohorts.
In both cohorts, risk of receiving nephrotoxic medications appeared to be higher for patients with age < 65 years, women, or with CKD G3.
Prescription of nephrotoxic medications may reduce and iatrogenic kidney injury may be prevented with implementation of strategies to increase physician's awareness of patients’ CKD and knowledge of drug nephrotoxicity.
